• Consult A: 60 minute Initial Consult (or long follow-up consult): $577.50/*rebate $202.65
• Consult B: 40 minute follow-up consult: $385/*rebate $125.10
• Consult C: 30 minute follow-up consult: $288.75/*rebate $84.90
• Consult D: 20 minute follow-up consult: $192.50/*rebate $84.90
• Consult E: 15 minute script review or referral request consult: $144.50/*rebate $43.90
• Telehealth consults: rebate of $43.90 is applicable for 15/20/40 minute telehealth consults following at least one face-to-face consult in the prior 12 months; rebate of $202.65 available for a 60 minute follow up video consult

*Medicare rebates only applicable if consult is in accordance with Medicare Benefits Schedule.

Bulk billing not available.

Consumables (dressings/surgical equipment) attract a private out of pocket fee.

Cancellation policy: 24 hours notice is required for a refund (minus merchant booking fee absorbed by Your Integrated Health)

Your Integrated Health is not your usual General Practice! 

It is a specialised general practice designed to get to the root causes of your problems, which takes time in consultation, examination, investigation, accessing and assessing past medical information, and of course ultimately management, including careful explanation and education.

• 60 minute initial consult allows adequate time for your initial assessment
• When choosing your review appointment please be mindful of what needs to be covered- you will be advised at your initial appointment what length review appointment to choose.
• The same applies for any other subsequent appointments 

• Location: One Plus One Offices, Level 2, Suite 41/5 Everage St, Moonee Ponds, 3039
• Can be tricky to find - with google maps unhelpful; entry is between Moonee Central Vet and Connect Hearing
• Parking: Limited 1 and 2 hour parking spaces in Hall St and Everage St; Undercover Coles Car Park (Taylor St); Aldi Car Park (Taylor St); Disabled parking spots available in Hall St (one way traffic, enter from Margaret St)
